This Privacy Notice describes the information Collabrios Health Holdings, LLC (“Collabrios”, “we”, “our”, or “us”) collects, how we use and retain this information, with whom we share it, and the choices you have in connection with these activities. This Privacy Notice applies to collabrioshealth.com, a website owned and operated by us (the “Site”). This Privacy Notice does not apply to the Collabrios’ Electronic Medical Record or Third-Party Administrator platforms.

  1. PERSONAL INFORMATION WE COLLECT, HOW WE USE IT, AND HOW WE SHARE IT

When you interact with our Site, we collect your personal information. Personal information is information that identifies, relates to, describes, is capable of being associated with, or could reasonably be linked, directly or indirectly, with a particular individual or household.

  1. When you visit our Site:
  • Contact Us. When you complete the “Contact Us” form on our Site, we will collect your identifiers (name, email address, phone number), employment information (employer name), and any other information you wish to include in the “Comments” field. This information will be shared with our customer relationship management platform to assist us with efficiently and effectively communicating with you. We will use this information to respond to your message and generally correspond with you about the Site and our services.
  • Navigate the Site. In addition to the personal information you provide, we may also collect internet and other network activity information automatically via pixels and cookies to: (i) provide the Site; (ii) enhance user experience; (iii) conduct analytics to improve the Site; (iv) prevent fraudulent use of the Site and detect unlawful activity; (v) diagnosis and repair Site errors, and, in cases of abuse, track and mitigate the abuse. In general, to disable cookies and limit the collection and use of information through them, you can set your browser to refuse cookies or indicate when a cookie is being sent. When you opt-out an opt-out cookie will be placed on your device. The opt-out cookie is browser and device specific and will only last until cookies are cleared from your browser or device.

Generally, these service providers perform services on our behalf such as data storage, security, fraud prevention, customer relationship management, and IT.
  • Affiliates and Subsidiaries. We share your personal information within Collabrios, including with our affiliated organizations for the legitimate business purposes of efficiently and effectively providing the Site.
  • For legal purposes. We will share your personal information when we are legally required to do so, such as in response to court orders, law enforcement or legal process, including for national security purposes; to establish, protect, or exercise our legal rights, as required to enforce our terms of service or other contracts; to defend against legal claims or demands; to detect, investigate, prevent, or take action against illegal activities, fraud, or situations involving potential threats to the rights, property, or personal safety of any person; or to comply with the requirements of any applicable law.
  • Business Reorganization. In the event that we enter into, or intend to enter into, a transaction that alters the structure of our business, such as a reorganization, merger, acquisition, sale, joint venture, assignment, consolidation, transfer, change of control, or other disposition of all or any portion of our business, assets or stock, we would share personal information with third parties, including the buyer or target (and their agents and advisors) for the purpose of facilitating and completing the transaction. The Site may contain links to third-party websites or third-party plug-ins (e.g., Facebook, Instagram, and X/Twitter). If you choose to use these third-party links and features, we are not responsible for the content or practices of such third-party websites or services. The collection, use, and disclosure of your information will be subject to the privacy policies of the third-party websites or services, and not this Privacy Notice. We urge you to read the privacy and security policies of these third parties.

  1. CHILDREN’S PRIVACY

The Site is intended for adults only. We do not knowingly solicit any information from individuals under the age of sixteen (16) years old, nor do we knowingly market or otherwise target our Site or its services to children. If we become aware that a visitor to our Site is under the age of sixteen (16) years old, we will remove his or her personal information from our files.

  1. DO NOT TRACK 

Do Not Track (“DNT”) is a privacy preference that users can set in certain web browsers. We do not currently recognize or respond to browser-initiated DNT signals.

We take measures to protect your information against accidental or unlawful destruction or accidental loss, alteration, unauthorized disclosure, or access. However, no method of transmission over the Internet, and no means of electronic or physical storage, is absolutely secure.
