Collabrios Community Care connects government, community, provider, and healthcare partners to coordinate services that help people live safely and independently, spanning aging and disability agencies, HCBS and LTSS programs, community-based organizations, Adult Protective Services, Ombudsman and Adult Day Services, caregiver programs, and Medicaid community programs.

Collabrios Health brings together three companies that built technology for PACE and community care from the ground up: RTZ Systems, a pioneer of PACE-specific EM Rs; PeerPlace Networks, a trusted partner to government and community-based organizations; and the PACE EMR division of Tabula Rasa HealthCare.
Today, that experience comes together in CollabriosONE for PACE and Collabrios Community Care, backed by HITRUST el certification and SOC 2 Type II audits.
